The BBC engaged in a "major breach" of broadcasting rules by not revealing that the narrator of a programme about Gaza was the child of a Hamas representative, as ruled by regulatory body Ofcom.
An investigation by the media regulator into the programme Gaza: How to Survive a Warzone has determined that the programme was "materially misleading".
The BBC's director general had earlier expressed regret, saying there had been "a major oversight in relation to accuracy".
Ofcom has instructed the broadcaster to transmit a evening declaration about its findings.
"Given this constitutes a significant violation of our regulations, we are ordering the broadcaster to broadcast a message of our determinations against it on the BBC's second channel at 9pm, with a date to be finalised," the statement read.
'Risk to Damage Confidence'
The watchdog's statement stated: "Our probe determined that the documentary's failure to disclose that the presenter's male guardian held a role in the Hamas-run administration was substantially inaccurate.
"It meant that the audience did not have vital facts which may have been very significant to their assessment of the voice and the information he provided.
"Confidence is at the heart of the bond between a media organization and its audience, notably for a national broadcasting corporation such as the British Broadcasting Corporation.
"This error had the risk to erode the exceptionally strong degrees of trust that the public would have put in a BBC factual programme about the Gaza conflict."
The programme was removed from the BBC's streaming service in the month of February after the boy's familial connections were revealed, and in this past July an corporation's own inquiry determined it violated the broadcaster's content policies on factual correctness.
A corporation official stated: "The regulator's decision is in accordance with the conclusions of the corporation's inquiry, that there was a major oversight in the programme in relation to the BBC's Editorial Guidelines on truthfulness, which aligns with clause 2.2 of the authority's broadcasting regulations.
"We issued an apology for this and we comply with the regulator's ruling in its entirety. We will comply with the penalty as soon as the schedule and wording are confirmed."
